Iron Overload in Dialysis Patients: Rust or Bust?
نویسنده
چکیده
A recent study using magnetic resonance imaging (MRI) to assess liver iron content (LIC) by Issad and colleagues provides observational data from prevalent peritoneal dialysis patients, showing that few have iron overload. These results complement earlier MRI studies in hemodialysis patients by Rostoker et al. and other groups showing high MRIestimated LIC in hemodialysis patients and a high incidence of moderate to severe iron overload. The discussions in these reports, and in several editorials and commentaries, have warned that our present i.v. iron use practices are overloading hemodialysis patients and carry great risk. Anemia is a major problem in most patients receiving chronic dialysis, and treatment with i.v. iron was necessary in some even before the approval of epoetin in 1989. Fewer than 10% of patients in this era were transfusion dependent, but over the years these patients developed iron overload. In 1 study of 12 such patients, ferritin ranged from 3000 to 19,360 mg/l, transferrin

Using Iron-Chelating Agents in Critically Ill Patients with Iron Overload. Fact or Fiction?
Recently, some evidence has shown that the failure of iron homeostasis may occur in critically ill patients and can lead to iron overload. Elevated ferritin levels as a body iron burden index in critically ill patients may be associated with depressed level of consciousness and greater mortality. However, the necessity of using iron-chelating agents in clinical situation is still unknown for th...
متن کاملAssessment of Pancreatic Iron Overload in Transfusion Dependent Thalassemic Patients
Advances in the management of transfusion dependent thalassemic patients have improved the survival of these patients. The most important consequence of repeated and frequent transfusions is iron accumulation in vital organs. The magnetic resonance imaging (MRI) is a non-invasive and valid technique for the estimation of iron stores. Despite multiple studies about cardiac and liver MRI T2*, the...
متن کاملIron overload as cardiovascular risk factor in children and adolescents with renal disease.
BACKGROUND Iron overload can affect cardiac structure and function by the production of free radicals in addition to iron deposits in heart muscle. The purpose of this study was to compare traditional and non-traditional cardiovascular risk factors (CVRF) in children and adolescents on renal replacement with and without iron overload. Also, we evaluated the relationships between iron overload a...
متن کاملIron Inhibits Respiratory Burst of Peritoneal Phagocytes In Vitro
Objective. This study examines the effects of iron ions Fe(3+) on the respiratory burst of phagocytes isolated from peritoneal effluents of continuous ambulatory peritoneal dialysis (CAPD) patients, as an in vitro model of iron overload in end-stage renal disease (ESRD). Material and Methods. Respiratory burst of peritoneal phagocytes was measured by chemiluminescence method. Results. At the hi...
متن کاملImpact of iron sucrose therapy on leucocyte surface molecules and reactive oxygen species in haemodialysis patients.
BACKGROUND It has been suggested that iron increases oxidative stress and that an excess of iron contributes to cardiovascular disease and infections in haemodialysis patients. In the present study, the effects of parenterally administered iron on leucocyte surface molecule expression and the production of reactive oxygen species (ROS) were evaluated. METHODS Ten chronic haemodialysis (HD) pa...
